AFC Simpli Easy Form

AFC Simpli Easy Form was created for the beginner programmer who would like to have a simple contact form on their website that will send the owner the persons name, email, phone number and includes a state drop down list, plus more. The form includes email validation and will display the error on the form should the form not be filled out properly. If the form is correctly filled out it will goto a Thank You page. You can modify the output to your own Thank You very easily. The AFC Simpli Easy Form is easy to modify for your needs. All you need to do is add your email address into the output.php file in two places and upload the files to your web hosting server.

AFC Simpli Easy Form Includes all the files you need. You can edit anything you need to make this form customized to serve you better. There are no restrictions to this script so feel free to add or modify the code to suit your needs
